Rights and Reproductions Request Form

The information requested below will assist us in determining the feasibility of your order. Submission of the request does not guarantee SAM's ability to fulfill the order nor does it obligate agreement by the applicant to SAM's related terms and conditions.

You will receive a Rights and Reproductions Agreement by email, unless requested otherwise. You will also be provided with an invoice summarizing all fees, including any applicable rush fees. Please follow the instructions contained therein to finalize the order. In the event that the order cannot be fulfilled, notification will be sent to that effect.
