Seattle Art Museum Presents SAM CAMP

MY CHILD AND I UNDERSTAND AND AGREE TO THESE TERMS:

My child will bring a bag lunch, beverages and snacks daily. I understand that refrigeration is not available.

I will be responsible for timely drop-off and pick-up at SAM Downtown, Olympic Sculpture Park or Seattle Asian Art Museum (see week for specific drop-off and pick-up locations). If my child has not been picked up by myself or someone that I have designated, SAM will assess a $60 late fee starting at 4 pm, prorated at $15 every 15 minutes.

If my child exhibits aggressive or extremely disruptive behavior, SAM staff have the right to request immediate pick-up from the camp. In this circumstance, my registration fee will not be refunded.

I give permission for my child and her/his artwork to be photographed, on display and possibly published in SAM's printed and online materials.

I give permission for my child to go on field trips at SAM Camp. This includes travel to SAM’s three sites—SAM Downtown, the Seattle Asian Art Museum and the Olympic Sculpture Park—and to other locations.

I agree to share any special needs my child might have on the emergency contact form. I will list any known disabilities that SAM staff should be aware of (e.g., asthma, ADHD, dyslexia).
